DOT MEDICAL CARDS - OPSI Required Physicals to Get or Renew Hoisting Licenses What is a DOT Medical Card?  A DOT Medical Card is a required card by OPSI and by the Federal Motor Carrier Safety Administration (FMCSA) for hoisting operators and commercial motor vehicle (CMV) drivers. In order to receive a DOT Medical Card you must first get a DOT physical to determine whether you are physically, mentally and emotionally fit to operate Heavy Equipment or a Commercial Vehicle.
