Hoisting License Application Information

PLEASE NOTE: The Department of Public Safety has made a name change to the Office of Public Safety and Inspections (OPSI), and effective July 2, 2018 will have a new address:

Come to any hoisting class we offer and we will give you the test applications & help you fill them out properly. We look forward to working with you.

A. There are several locations for testing and the state will pick the one closest to you. You can not pick the location of your test. You will need to wait for the state to contact you.

B. You need a valid drivers license/copy, approved medical card/copy, check for $75.00 per test (made out to Commonwealth of Mass), completed application. Mail all to address on application.

C. The state is behind on processing, so be patient. (If you have forgotten anything they will contact you).

E. My competition claims only they can get you a seat for two tests in one day. The fact is anyone gets two tests in one day.
(Pay $75.00 fee for each license type you want) Ex: 2A & 1C - $150.00
